RESUMO

Field edge habitat in homogeneous agricultural landscapes can serve multiple purposes including enhanced biodiversity, water quality protection, and habitat for beneficial insects, such as native bees and natural enemies. Despite this ecosystem service value, adoption of field border plantings, such as hedgerows, on large-scale mono-cropped farms is minimal. With profits primarily driving agricultural production, a major challenge affecting hedgerow plantings is linked to establishment costs and the lack of clear economic benefits on the restoration investment. Our study documented that hedgerows are economically viable to growers by enhancing beneficial insects and natural pest control and pollination on farms. With pest control alone, our model shows that it would take 16 yr to break even from insecticide savings on the US$4,000 cost of a typical 300-m hedgerow field edge planting. By adding in pollination benefits by native bees, where honey bees ( Apis mellifera L.) may be limiting, the return time is reduced to 7 yr. USDA cost share programs allow for a quicker return on a hedgerow investment. Our study shows that over time, small-scale restoration can be profitable, helping to overcome the barrier of cost associated with field edge habitat restoration on farms.

BACKGROUND: Pulmonary dirofilariasis is a rare entity caused by Dirofilaria immitis, the dog heartworm that is transmitted to humans by mosquitos. This filarial nematode enters the subcutaneous tissue, travels to the right ventricle where it dies, and then embolizes the pulmonary vasculature, causing a small pulmonary infarction, which subsequently appears as a solitary nodule. Although these nodules are usually identified incidentally by chest radiography in asymptomatic patients, the lesion is generally presumed to be neoplastic. Diagnosis is made by surgical excision. METHODS: We reviewed the pathology reports at Memorial Medical Center, Savannah, GA since 1990. RESULTS: There have been approximately 150 cases reported in the literature worldwide. We found 4 patients with pulmonary dirofilariasis diagnosed at our hospital since 1992. A review of the pathogenesis of the disease and clinical manifestations, diagnosis, and treatment of this entity are presented. CONCLUSIONS: Pulmonary dirofilariasis is a benign condition, transmitted by mosquitos to humans that results in peripheral pulmonary nodules. Awareness of this entity is important in the differential diagnosis of pulmonary coin lesions.

The authors conducted a study to determine whether basic instruction in reading chest radiographs could enable flight nurses to interpret radiographs accurately. Flight nurses were taught 10 trauma-related chest roentgenographic patterns. The chest radiographs of 40 transported trauma patients were chosen randomly over a 14-month period for interpretation by a flight nurse and a referring physician. Flight nurses correctly identified 47 out of 55 patterns (85%). Referring physicians correctly identified 37 out of 55 (67%). Flight nurses also correctly identified 13 patterns that the referring physicians missed. The results indicate that emergency radiologists can advance and improve the radiological aspects of emergent patient care by providing flight nurses with minimal training in interpreting radiographs. When a flight nurse and a physician in an outlying area--where trauma is less commonly encountered--work as a team, more accurate and earlier diagnoses can be made, and therapy can be instituted earlier.

RESUMO

We have presented the case of a 32-year-old man who sustained blunt trauma to the chest in a motor vehicle accident. Plain roentgenograms showed a widened mediastinum and pneumomediastinum, and an esophagogram with water-soluble contrast material showed an esophageal laceration at the T-4 level.

Metastatic carcinoma to the thyroid gland rarely is encountered in clinical practice; however, autopsy series have shown that it is not a rare occurrence. A case of adenocarcinoma of the colon with metastases to the thyroid is reported. A review of the literature reveals that melanoma, breast, renal, and lung carcinomas are the most frequent tumors to metastasize to the thyroid. Metastatic disease must be considered in the differential diagnosis of cold nodules on radionuclide thyroid scans, particularly in patients with a known primary.

A stabilate prepared from Babesia bovis-infected Boophilus microplus ticks was used to infect intact adult cattle. Whole sera and immunoglobulin fractions from representative sera were tested by complement fixation (CF), indirect fluorescent antibody (IFA), and antibody-dependent cell-mediated cytotoxicity (ADCC) assays. The last test utilized 51Cr-labeled chicken erythrocytes coated with Babesia bovis antigen as targets. Mononuclear cell preparations, obtained from peripheral blood of normal donors and consisting of lymphocytes with 2--6% large monocytes, were used as the source of effector cells. Antibody activity was detected by all tests between 14 and 16 days following infection. Specific IgM and IgG1 were reactive in both CF and IFA tests, although the development of high titers was attributable to IgG, alone. The ADCC activity was restricted to IgG1 fractions and was greater in those sera or fractions with greater CF activity. No activity was demonstrated in IgG2 fractions by any test used.

RESUMO

Acute hematogenous osteomyelitis was detected in a 23-year-old sickle-cell patient after standard radiographs of the right femur showed gas within the medullary cavity. Three anaerobic organisms were cultured from surgically aspirated materials, but no source of infection was found. After surgical drainage and six weeks of intravenous clindamycin therapy, there was clinical improvement and radiographic resolution of the gas formation.

A cryopreserved stabilate was prepared from Babesia bovis-infected Boophilus microplus ticks. The stabilate was used to infect a group of mature Bos taurus-Bos indicus cows. The survivors of the initial infection were rechallenged with the homologous stabilate 65 days later. All cows reacted parasitologically after primary challenge, and most responded serologically. With the exception of two cows, those not responding serologically died of acute babesiosis. The two that survived the primary challenge, but did not produce detectable antibody, died after secondary challenge. All serologically-positive cows survived the secondary challenge. Complement fixation and indirect fluorescent antibody tests were performed on all cows' sera and, in addition, immunoglobulin fractions were prepared and analyzed from selected sera. Both IgM and IgGl were important in the initial response. A secondary IgM response was noted by complement fixation assays. No activity was detected in any IgG2 fraction.

One hundred fifty-nine Holstein calves were imported into St. Lucia from the U.S.A. An outbreak of babesiosis occurred 17 days post-arrival, and an outbreak of anaplasmosis occurred 5 months after importation. Sera obtained 3, 6, and 12 months post-importation revealed a high prevalence of IFA titres to Babesia bovis and B. Bigemina 3 months after arrival and an increase in titres to Anaplasma marginale 6 months after arrival. Sera obtained from native cattle from several places on the island indicated infection rates of 80, 65 and 64% with A. marginale, B. bigemina and B. bovis, respectively. The rapid card test only indicated a 25% prevalence of infection of native cattle by A. marginale. This low prevalence was probably due to deterioration of serological activity during shipment.

RESUMO

Metastatic pulmonary nodules from a well-differentiated hepatoma were well demonstrated by a Tc-99m-labeled hepatobiliary imaging agent. Uptake of this type of radionuclide by metastases from a primary hepatic malignancy has not previously been reported. Such radiopharmaceuticals may prove useful in the future as imaging agents for primary hepatobiliary tumors.

Results of complement-fixation (CF), indirect fluorescent antibody (IFA), and card agglutination (CT) tests were statistically compared, using 380 serum samples obtained from 140 cattle which were disease-free or naturally or experimentally infected with Anaplasma marginale of Colombian origin. The IFA test was significantly the most sensitive for detection of amimals infected with anaplasmosis (97%); the CT test and the CF test were less so (84% and 79%, respectively). However, the most efficient test for identifying noninfected animals was the CF test (100%), and the CT and the IFA tests were less efficient (98% and 90%). A linear regression analysis performed on the average IFA and CF titers of 10 calves artificially infected with A marginale during a 20-week period showed significant regression coefficients for both tests. The regression line for the CF titers decreased below the sensitivity threshold at 14 weeks after calves were inoculated, whereas the regression line for the IFA titers continued above the sensitivity threshold 20 weeks after inoculation. The CT test also detected antibodies until the end of the observation period.

RESUMO

Metastatic serosal and omental implants from a primary papillary endometrial carcinoma were well demonstrated by radionuclide uptake on a bone scan utilizing 99mTc-methylene diphosphonate. Imaging of an endometrial malignancy by a 99mTc-labeled phosphate compound has not previously been reported. While the exact mechanism of accumulation in such soft-tissue neoplastic foci is not known, several possibilities have been suggested. In this particular case, the mechanism appears related in some way to the presence of dystrophic calcifications within the metastatic deposits.

RESUMO

The value of 67 Ga-citrate scanning in cases of gastrointestinal malignancies is discussed. Seven cases are presented, including lymphomas of the stomach, small bowel, and rectum, and adenocarcinomas of the stomach and colon. In a review of the literature, there is general pessimism regarding the use of 67Ga scans in GI malignancies. Based on previous reports and our own experience, specific clinical situations are cited in which the scan is of considerable value for diagnosis and followup of GI malignancy.

A total of 372 serum samples were collected from Colombian cattle before and during the course of natural Babesia spp. infection on the North Coast of Colombia. The serum samples were used to compare indirect fluorescent antibody (IFA) with complement fixation (CF) tests for diagnosis of babesiosis. The IFA technique detected Babesia argentina antibodies an average of 4.0 weeks earlier than the CF test and Babesia bigemina an average of 2.5 weeks earlier. Both IFA and CF were capable of differentiating B. argentina and B. bigemina infections, however in some cases cross reactions were observed. In general IFA titers were at relatively high levels of 1:640 to 1:5120 in comparison with CF titers of trace to 1:80. In cases of mortality due to babesiosis, both IFA and CF serologic techniques were very useful in indicating the cause of death. Although both IFA and CF are laboratory tests, the IFA technique had advantages over the CF in simplicity, economy and speed of performance.

An analysis of one year's experience at our institution with 781 cerebral radioangiograms is reported. In patients who had follow-up radioangiographic evaluation, an overall accuracy of 88% was achieved using the anterior method. Accuracy in detecting carotid occlusive lesions alone was 83%. Detection of subdural hematomas by radioangiography was exceptional. Our results correlate well with those of others in detection of cerebrovascular occlusive disease, other vascular lesions, and avascular lesions. In our experience, the extremely high yield of information and the simplicity of performing radioangiography of the head and neck justify its routine use in all cases referred for brain scanning.
